In addition this year we are introducing a new approach to managing long term conditions for many of our patients. Our new approach will include the production of an annual care plan highlighting agreed goals. Further details are available from the menu on this website

The out-of-hours service is provided by Yorkshire Ambulance Service. To access this service, you can ring 0845 056 8060 where details will be given.

You may be asked to make an appointment at the surgery during normal hours. This does not give you priority when making the appointment.
