We are currently altering the way we recall patients with Long Term Conditions(eg Diabetes, Asthma). This might mean your annual review could be slightly earlier or later than previously

In addition this year we are introducing a new approach to managing long term conditions for many of our patients. Our new approach will include the production of an annual care plan highlighting agreed goals. Further details are available from the menu on this website  

Family Information Service

 

The Families Information Service Hub (FISH) is a free and impartial information service for children, young people, families and the people who support them.

How can FISH help?

FISH is a one-stop-shop for details of services in the East Riding for children and young people aged 0-20.

Talk to us about:

  • Early Learning and Childcare - from September 2010 all 3 and 4 year olds who are eligible for the early learning and childcare places will be entitled to 15 hours per week. This is an increase from the existing 12.5 hours. From last September 2009 some childcare settings have offered 15 hours as part of a pilot. For more information and a list of local settings please telephone FISH.

  • Paying for childcare - advice on working tax credits, childcare tax credits, early learning and childcare for three and four year olds and employer childcare vouchers
